Print this page using your browser’s Print menu, complete and sign it, then upload a scan or clear photo. Complete one form per catch. Do not include your child’s name, identifying information, catch photos, or identification documents on this form.
NOTICE TO THE PARENT OR LEGAL GUARDIAN
Big Uns would like permission to receive and review a catch you submit on behalf of your child under 13. We collect your name, email, signature, and consent record to request and verify permission and contact you about the submission. Marketing emails require a separate opt-in.
After consent is approved, you may provide an optional child first name or display name, fish name, weight, lake/body of water, lure, time of day, catch story, and photos. Only provide information needed for the story. Do not provide the child’s last name, contact information, social media handle, home address, or precise location coordinates. Include only photos you have permission to share.
We use these details to review the catch and consider creating a trading card. Selection is not guaranteed. Only if authorized below, selected fish images and approved catch details may appear on physical cards distributed to collectors and public card-story pages reached by QR codes. Recognizable child images in promotional materials require the separate optional permission below. Your contact details and signed form are private.
Our service providers help store and process this information: Vercel (website hosting), Supabase (database and private files), Cloudflare R2 (private backups), Resend (submission emails), Sentry (error monitoring), and Cloudflare Turnstile (abuse prevention). Contact Big Uns using the details above for questions about these providers or your information. We do not sell personal information.
You may request access, correction, deletion, or withdrawal of consent by contacting hello@bigunstradingcards.com. We take reasonable steps to verify parental requests. Already distributed physical cards cannot be retrieved; withdrawal affects future collection and use subject to applicable requirements.
RETENTION
Unused consent requests expire after 30 days and are deleted within 7 additional days. Declined or unselected child submissions are deleted within 90 days of the decision. Selected catch content is retained while its card-story page is active, then deleted within 90 days. Signed consent records are retained during authorized use plus 3 years. Deleted information ages out of backups within 90 days.
